I, _________________________________________________, the enrolled participant and/or parent/legal guardian of the participant agree and understand that swimming is a HAZARDOUS activity. I also recognize that there are risks inherent in the sport of swimming, including but not limited to paralyzing injuries and death.

The participant and/or parent/legal guardian of the participant hereby agrees to participate in the swim lessons and other aquatic activities and hereby agrees to indemnify and hold harmless BUBBLES SWIM SCHOOL, INC. its coaches, officers, directors, agents and employees against any liability resulting from injuries that may occur to the participant while participating in swim lessons and other aquatic activities. The participant and/or parent/legal guardian of the participant also agrees to indemnify BUBBLES SWIM SCHOOL, INC. for any damages incurred arising from any claims, demands, action or cause of action by participant.

The participant and/or parent/legal guardian of the participant authorizes any representative of BUBBLES SWIM SCHOOL, INC. to have the participant treated in any medical emergency during their participation in swim lessons or other aquatic activity. Further, the participant and/or parent/guardian agree to pay all costs associated with medical care and transportation of the participant.

I have noted on the back of this form any medical and or health problems of which the staff should be aware.

POLICIES

1. Make-up lessons are limited to one make-up per session for all classes. Make-ups must be made up during the current session. Calling in advance to notify us of your absence aids us in finding make-up classes for others. Additional requests for make-ups must be accompanied by a valid doctor's note. Requests will be granted if space is available.

2. No refunds or cancellations: Prolonged illness or injury may entitle you to a voucher for remaining lessons. Vouchers are only issued from the time of contact forward. No voucher will be issued for weeks that have passed. Requests for vouchers must be in writing and require a valid doctor's note. All requests await the approval of the swim school owner/director.

3. Rain/Inclement weather: Lessons may be conducted during light rain, however students will be promptly removed at the first sign of lightning or thunder. Cancellations made by the school will result in re-scheduling or additional make-ups issued. No refunds for school closings.

4. Cancellations made by the school will result in a voucher or a make-up. No refunds will be issued for cancellations made by the school.

5. Student/Instructor Ratios - Due to the nature of certain classes, student/instructor ratios may be increased. Ratios may change in a particular class if a make-up is scheduled.

6. Parents must remain on the premises during their child's lesson.
   
   I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AGREEMENT AND HAVE CAREFULLY CONSIDERED THE  STATEMENTS REGARDING BUBBLES SWIM SCHOOL POLICIES.

          I SIGN THIS FORM HAVING FULL KNOWLEDGE OF ITS CONTENTS AND SIGNIFICANCE.
